Eligibility Center

You must register with the Eligibility Center to be certified as eligible to practice, compete, and receive an athletic scholarship during your first year at a Division I or II school. In addition, you will answer questions regarding your amateurism status. Things you would need to provide when registering with the Eligibility Center is below.

  • Valid email for student- a valid email is needed to check your status throughout the process.
  • Student Personal Information – This includes date of birth, contact information, and address.
  • Student education history – information regarding secondary and high schools you have attended will be required.
  • Student sports participation history – Information on the sport(s) you plan to participate in NCAA. Also, answering questions regarding your amateurism.
  • Payment – Registration fee for U.S., U.S. territories (includes American Samoa, Guam, Northern Mariana Islands, Puerto Rico and U.S. Virgin Islands) and Canada is $90. International student's registration fee is $150.

Fill out College Application

It is best to apply to multiple colleges and to start applying for colleges the start of your senior year. The reason is you are giving colleges time to make their decision on your acceptance. In addition, there are deadlines to applying for colleges. Applying early allows you to apply for other colleges if your top choices were denied. A college application form are usually online applications, but paper forms are accepted as well. The application form is one part of the process and would need to submit the remaining documents. Below is a list of the documents needed.

  • Application Forms
  • Application Fee ($30 - $90)
  • Your High School Transcript (unofficial submitted with application)
  • Final Transcript (submitted by high school)
  • Admission Test Scores (ACT or SAT scores)
  • Essays/Letter of Recommendations (Not required depending on school)

Fill out FAFSA

FAFSA is the Free Application for Federal Student Aid which is a form you need to fill out to receive financial aid from the federal government to help pay for college.

The FAFSA sections that would need to be filled out are the student eligibility, school section, dependency determination, parent demographics, financial information, and sign & submit.

Final Transcripts sent to University and Eligibility Center

Once graduated high school, the University and Eligibility Center would need your final transcript and proof of graduation. You can send your high school transcripts to the Eligibility Center by electronically sending your transcript to your account through the High School portal. You can also send it by mailed but having it uploaded would be much quicker. You can ask your high school to have it uploaded through one of the listed providers below.

Immunization Forms & Medical Records

Students who attend University schools are required to provide copies of immunization records to the school health services department. If not, then the student could be dropped from classes or have a hold on their account. In addition, college athletic departments may ask for medical records to see if the student-athlete had any prior injuries or surgeries.
